You can deliver the filled in form at the practice or send it to us by (e)mail. Upon registration you will have to identify yourself with a valid proof of identity.

If you have any health problems during your holidays or temporary stay in our region you are also welcome for a consultation. If you have a Dutch health insurance, we send the invoice to your insurance company. In other cases you need to pay cash (no bank card or credit card payments accepted). Depending on the duration of the consultation, this will cost you between €29,31 and €58,62.
